Creekside Village in Mishawaka, IN is hiring a Social Services Assistant to provide medically related social services to residents under the supervision of the Social Services Director. You will support residents' well-being and coordinate with the care team.

Ideal candidates bring a passion for geriatric advocacy, strong interpersonal skills, and teamwork. Previous social services experience is preferred, and CNA certification is a plus.

Qualificações

• High School Diploma; Social Services Designee Certificate preferred
• Passion for geriatric advocacy and commitment to senior care excellence
• Previous social services experience preferred

Responsabilidades

• Provide medically related social services to residents under supervision of the Social Services Director
• Support residents' physical, mental, and psychosocial well-being
• Collaborate with interdisciplinary teams and families
• Advocate for residents and assist in care planning
• Maintain compassionate, professional relationships with residents and staff
