Job summary

WeCare at Jameson Care Center in New Castle, PA is seeking a Social Work Assistant to support residents and families with care planning and coordination. This role collaborates with nursing and interdisciplinary teams to ensure a high quality of life for residents and to support staff in daily operations.

Qualified candidates hold a Bachelor’s Degree in Social Work or Human Services with geriatrics or long-term care experience preferred.

Qualifications

• Bachelor’s degree in social work or human services required.
• Experience in geriatrics or long-term care preferred.
• Ability to work effectively with interdisciplinary teams.

Responsibilities

• Reviews and follows up on concerns for residents and staff with incident reports.
• Collaborates with caregivers and departments to improve residents’ living experience.
• Keeps supervisor informed of health and safety-related occurrences.
• Oversees care plans, MDS, and quality improvement processes.
• Keeps administration informed of stats, progress and achievements.
• Work tactfully with personnel, residents, family members, visitors and the public.
• Work well with interdisciplinary teams.
• Other duties as assigned.
