Logan Health in Kalispell, Montana seeks a Board Certified/Board Eligible Internal Medicine or Family Medicine physician to provide medical oversight for patients in our Skilled Nursing Facility and Long-Term Care programs.

The role offers care for a medically complex geriatric population, collaboration with an experienced interdisciplinary team and dedicated APN, and a focus on post-acute and chronic disease management in a supportive community hospital setting.

Qualificações

• Board Certified/Board Eligible Internal Medicine or Family Medicine physician.
• Experience in geriatric or long-term care a plus.
• Ability to lead an interdisciplinary team.

Responsabilidades

• Provide medical oversight for short-term rehabilitation patients and long-term care residents.
• Deliver post-acute and chronic disease management.
• Help reduce avoidable hospital readmissions and ED visits.
• Collaborate with nursing, rehabilitation, case management, social work and specialty providers.
• Participate in care conferences, family meetings and goals-of-care discussions.
• Coordinate transitions between hospital, skilled nursing, long-term care, specialty and community providers.
