Job summary

Sauk County Health Care Center is seeking a Staff Nurse to provide registered or licensed practical nursing care to residents in a skilled nursing environment. You will work on PM or NOC shifts and collaborate with physicians and a multidisciplinary team to deliver person-centered care.

The role requires an Associate's degree in Nursing and an active RN license, with preference given to candidates with a Bachelor’s degree and geriatric experience.

Qualifications

• Associate's degree in Nursing is required.
• Registered Nursing (RN) license is required.
• Bachelor’s degree in Nursing preferred.
• Geriatric nursing experience preferred.
• Minimum 2 years of nursing experience preferred.

Responsibilities

• Promote or restore residents' health by applying nursing processes.
• Provide physical and psychological support to residents and families.
• Adhere to nursing standards and state regulations and maintain medical records.
• Administer prescribed treatments and medications.
• Supervise CNAs and assist with care plans and rounds.
• Document resident information in ECS and MDS systems.
