Job summary

Sauk County Health Care Center is seeking a Staff Nurse to provide registered or licensed practical nursing services for residents. Shifts include PM and NOC, with standard hours subject to change. The role emphasizes resident-centered care, collaboration with physicians, and adherence to nursing standards.

The position requires an Associate's degree in Nursing and an active RN license; a Bachelor's degree is preferred. Experience with geriatric populations and CNAs supervision is valued.

Qualifications

• RN with an associate's degree in nursing required.
• RN license required.
• Gerontic nursing experience preferred (2 years).

Responsibilities

• Promote or restore residents' health by completing the nursing process and collaborating with physicians and multidisciplinary team members.
• Provide physical and psychological support to residents and their families.
• Promote residents' independence with a resident-centered model of care.
• Assure quality of care by adhering to standards and state nurse practice acts.
• Administer prescribed treatments and medications and monitor responses.
• Assist with admission of new residents and supervise CNAs as appropriate.
• Accompany physicians on rounds and update on residents’ conditions.
• Document resident information in ECS and enter data in MDS as needed.
• Assist in developing resident care plans and monitor adherence.
• Record progress and changes in residents' condition to maintain clinical records.
• Monitor and record vital signs and document procedures and medications.
• Assist with transfers, repositioning, and answering call lights promptly.
• Prepare incident reports and participate in in-service training.
• Ensure policy adherence by staff and aid with inventory of supplies.
