Job summary

Element Care is seeking an experienced Registered Nurse to join the PACE interdiscliplinary team in Lynn, MA. You will provide direct geriatric nursing care, develop and revise care plans, and coordinate with physicians and caregivers to keep participants safe and independent.

Role is full-time, Monday to Friday, 8am-4pm, with good work-life balance and benefits. Travel locally is required, and a mask fit requirement applies where necessary.

Qualifications

• Registered Nurse with MA license.
• 2+ years of geriatric experience preferred.
• Proficient in Microsoft Office applications.
• Ability to travel locally in personal vehicle.

Responsibilities

• Provides direct care to participants and develops care plans.
• Collects medical and social information for decision making.
• Coordinates with Interdisciplinary Team for comprehensive plans.
• Monitors care delivery to assure quality and safety.
• Administers medications and supports home delivery where needed.
