Job summary

TruHealth in Dublin, GA is seeking an RN Case Manager to oversee care coordination for members in a Medicare Advantage program. You will visit patients in person or by phone to ensure proper nursing care, collaborate with physicians to resolve issues, and coordinate with health care teams to assess and plan care.

The role requires strong communication, teamwork, and adaptability, with responsibilities across assessments, care planning, education, and discharge coordination.

Qualifications

• Graduate of an accredited RN program.
• Minimum 2 years clinical nursing or geriatric rehabilitation.
• 2 years managed care experience required.
• 3-5 years case management in managed care preferred.
• Current RN license in the assigned state; CCM preferred.
• CPR for Healthcare Professionals certification; valid driver’s license.

Responsibilities

• Complete Health Risk Assessments for members as assigned.
• Initiate, update and revise care plans as needed.
• Maintain a caseload of patients as assigned.
• Coordinate with interdisciplinary team to plan and evaluate patient care.
• Educate patients and families using available teaching resources.
• Coordinate outpatient discharge planning based on patient needs and coverage.
• Participate in audits and generate reports for the Care Coordination program.
• Engage in ongoing professional development and program improvement.
