Job summary

TruHealth is seeking an RN Case Manager in Dublin, GA, to manage and support case management strategies for Medicare Advantage members. You will visit patients, coordinate with physicians and the health care team to assess, plan, implement and evaluate patient care plans.

Strong communication skills and the ability to adapt quickly to change are essential. Minimum 2 years of geriatric nursing or rehabilitation experience is required, with 2 years of managed care experience preferred.

Qualifications

• Graduate of an accredited RN program.
• Minimum 2 years of clinical nursing or rehabilitation experience in geriatric populations.
• 2 years of managed care experience required.
• CCM license preferred; CPR certification a plus.

Responsibilities

• Visit patients in person and/or by telehealth to ensure proper nursing care.
• Interviews with physicians to correct errors or omissions and to investigate questionable claims.
• Coordinate with health care team to assess, plan, implement and evaluate patient care plans.
• Maintain a case load and manage multiple priorities with minimal supervision.
