Job summary

TruHealth in Indianapolis, IN is seeking a Nurse Case Manager to oversee care coordination for Medicare Advantage members. You will visit patients, collaborate with physicians, and coordinate with the health care team to assess and implement care plans.

Strong communication and teamwork are essential for success in this role. The ideal candidate has nursing experience with geriatric populations, managed care exposure, and the ability to manage multiple priorities with minimal supervision.

Qualifications

• Minimum 2 years of clinical nursing or geriatric rehabilitation experience.
• 2 years managed care experience required.
• 3–5 years case management experience in a managed care environment preferred.
• Current RN license in assigned state; CCM preferred.
• CPR for Healthcare Professionals certification.

Responsibilities

• Complete Health Risk Assessments for assigned members.
• Initiate, update and revise care plans; maintain caseload.
• Coordinate care with interdisciplinary team to plan and evaluate patient care.
• Provide patient/family education based on identified learning needs.
• Coordinate outpatient discharge planning based on patient needs and benefit coverage.
• Participate in managed care audits; generate and maintain reports.
• Develop monitoring/collection tools and prepare audit findings.
• Educate members on health access options.
