Qualifications

• Uses nursing process (assessment, planning, implementation, coordination) to manage elder care.
• Directs staff performance and ensures compliance with nursing standards.
• Coordinates referrals and communication with health care team and community resources.

Responsibilities

• Follows nursing and case management processes for patient care.
• Manages referrals and coordinates with Indian Health Services, Medicare, Medicaid, and other resources.
• Documents in EHR/RPMS accurately; coordinates triage and scheduling; educates patients and families.
