Job summary

Leisure Glen Post Acute Care Center in Glendale, California is seeking a Part-Time Physical Therapist to join our Rehab Team. You will evaluate residents, design and implement treatment plans, and provide skilled therapy to a diverse elderly population to maximize mobility and independence.

This role emphasizes collaboration with physicians, nurses, and therapists, thorough documentation, and family education. California licensure and a minimum of 2 years healthcare experience are preferred.

Qualifications

• Current license to practice as a Physical Therapist in the state of California.
• Minimum of 2 years of experience in a healthcare setting, preferably in a skilled nursing facility.

Responsibilities

• Evaluate residents' physical abilities, functional limitations, and mobility needs using standardized assessments.
• Develop and implement individualized treatment plans based on assessment results, resident goals, and physician orders.
• Provide skilled physical therapy interventions, including therapeutic exercise, manual therapy, and modalities.
• Monitor and document resident progress and adjust treatment plans as necessary.
• Communicate with interdisciplinary team members to coordinate care and ensure optimal outcomes for residents.
• Educate residents and family members on physical therapy goals, treatment plans, and home exercise programs.
• Maintain accurate and timely documentation of physical therapy services provided.
• Participate in team meetings and quality improvement initiatives.
• Maintain current knowledge of physical therapy practice standards and regulations.
