Job summary

Vineyard Post Acute in Petaluma, CA is seeking a dedicated Physical Therapist (PT) for a Part Time/PRN role. The position offers pay starting at $70 per hour with DOE, flexible schedules, and a supportive rehab team focused on resident outcomes.

The PT will evaluate, develop, and deliver individualized treatment plans to restore function, improve mobility, and promote independence, while supervising PTAs and ensuring thorough documentation.

Qualifications

• Degree in Physical Therapy (Bachelor's/Master's/Doctorate).
• Active California PT license in good standing.
• Strong communication and collaboration with residents, families, and staff.
• Proficiency with EMR systems (Casamba, PointClickCare preferred).

Responsibilities

• Perform evaluations within 24 hours or 1 business day of referral.
• Develop and implement treatment plans with physician approval.
• Provide direct PT services per care plan and document progress.
• Supervise PTAs per state practice act and ensure accurate documentation.
• Participate in care conferences and interdisciplinary meetings.
• Provide caregiver and family training for safe discharge planning.
