Job summary

Odom Rehab, a division of Odom Health and Wellness (OHW), is hiring a Physical Therapist to provide outpatient services in geriatric rehabilitation. The role operates within assisted living facilities and community settings, offering a dynamic and growth‑oriented environment.

The PT will evaluate and treat patients, develop and update Plans of Care, supervise PTA/ PT aides, educate patients and families, and document daily progress.

Qualifications

• Graduation from an accredited Physical Therapy program with a degree in Physical Therapy.
• MN State Physical Therapy Licensure.

Responsibilities

• Provide outpatient physical therapy services following state guidelines.
• Evaluate and treat patients with targeted plans of care; reassess regularly.
• Oversee PT aide/ PTA under established plans of care.
• Deliver therapy including manual techniques, therapeutic exercises, balance training, and pain management.
• Educate patients and families on self-management and home programs.
• Document daily treatment and submit charges accordingly.
• Coordinate with physicians and medical staff; participate in staff meetings.
• Maintain licensure and continuing education requirements.
