Job summary

TruCare Living Centers in Columbus, TX is seeking a full-time Occupational Therapist to join our in-house rehab program. You will screen and treat patients with cognitive and ADL disorders, applying evidence-based practices to restore function and independence.

You will collaborate with a multidisciplinary team, document progress, and provide discharge planning while supporting families and caregivers in a supportive care environment.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice to support interventions.
• Provide rehabilitative, skilled and medically necessary treatment per regulations.
• Perform ADL retraining, therapeutic activities, exercises, modalities and device use as part of rehab.
• Assess progress and adjust treatments in the clinical record.
• Evaluate outcomes and modify services based on patient progress and referrals.
• Offer counseling to patients and families while coordinating with the health team.
