Job summary

TruCare Living Center in Selma, TX is seeking a full-time Occupational Therapist for its in-house rehab program. As part of our post-acute care team, you will assess cognitive and functional needs, design plans of care, and deliver evidence-based therapy to geriatric patients. New grads are welcome.

Qualifications

• OT license required.
• Open to all experience levels; new grads welcome.

Responsibilities

• Screen and evaluate patients with cognitive and/or ADL disorders and develop care plans.
• Use standardized assessments and evidence-based practice to guide interventions.
• Provide rehabilitative, skilled and medically necessary treatments.
• Administer ADL retraining; therapeutic activities; exercises; modalities; devices.
• Assess progress and adjust treatments in the clinical record.
• Evaluate treatment outcomes and refer as appropriate.
• Provide counseling to patients, families, and caregivers.
• Communicate with supervisor and health team about progress and plans.
• Provide discharge summaries per regulatory requirements.
