Job summary

TruCare Living Centers in Columbus, TX, is seeking a full-time Occupational Therapy Assistant to join our in-house rehab program. You will deliver rehabilitative, skilled treatment and participate in screening and evaluation of patients with physical functioning disorders in a post-acute setting.

As part of CAPLICO values, you will collaborate with a multidisciplinary team to support geriatric clients on recovery journeys, while ensuring compliance with regulatory practices.

Responsibilities

• Conduct screening and evaluation of patients with physical functioning disorders and/or wound care, developing care plans.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients in accordance with regulatory and clinical practice requirements.
• Administer procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Generate comprehensive discharge summaries of services provided, ensuring regulatory and clinical compliance.

Education

Physical Therapist license is required.
