Job summary

On Lok PACE is seeking a Rehabilitation Aide to provide functional maintenance therapy under PT/OT supervision for seniors enrolled in our program. You will help implement treatment plans, document progress, and assist with equipment maintenance in a community-based center.

Responsibilities include supporting therapy sessions, maintaining safety standards, orienting new staff, and delivering personal care as needed.

Qualifications

• Interest, ability, and at least one year of experience in working with the elderly or frail population.
• Interest, enthusiasm, skills, and ability to work in rehabilitation (therapy services).
• Ability to work independently and as part of the interdisciplinary team.
• Ability to use basic verbal and written English; bilingual preferred.
• Computer literacy for clerical, administrative, and/or clinical data tasks.
• Physical activities require the ability to stoop, reach, stand, walk, push, pull, lift, grasp, and/or other movements.

Responsibilities

• Provide Functional Maintenance Therapy (FMP) according to the individual treatment plan in therapeutic exercises, activities, groups, and other activities as directed by PT/OT.
• Record daily treatment service in the rehabilitation Monthly Service Record (FMP Flow Sheet).
• Prepare the rehab Monthly Service Record (FMP Flow Sheet) according to PT/OT treatment plans.
• Maintain the tidiness and cleanliness of therapy equipment and devices according to agency protocols.
• Monitor and check the rehab gym for adequate equipment and supplies; report malfunctions to therapists.
• Demonstrate safe practices by using proper body mechanics, universal precautions, and environmental safety.
• Promote the benefits of therapy and motivate participants in exercise programs.
• Orient and train new employees to FMP and equipment as requested by PT/OT.
• Provide direct personal care services to participants as needed and assigned by the supervisor.
• Contribute to center operations by leading, collaborating with interdisciplinary team, and supporting team plans.
• Other related duties as assigned by Program Manager or Therapists.
