InnovAge is seeking a Recreation Specialist II to plan and direct comprehensive activity programs for frail and elderly participants within its PACE day health centers.

Qualifications

• Bachelor’s degree in Therapeutic Recreation or a related field from an accredited college/university, or an Associate’s degree with 2 years of experience.
• Minimum one year working with frail or elderly populations.

Responsabilités

• Plans, organizes, directs and participates in activity programs tailored to frail and elderly.
• Evaluates participants’ functional status and conducts assessments.
• Develops and implements individual and group Recreational Therapy programs.
• Provides community resources per care plan and assists with ADL activities.
• Participates in Plans of Care with InnovAge PACE program participants.
• Coordinates treatment goals with other departments and maintains documentation.

Bachelor’s degree in Therapeutic Recreation or related field; Associates + 2 years experience
