Job summary

Pacific Coast Post-Acute is seeking a Registered Dietitian to oversee the nutritional care of residents in a 149-bed skilled nursing facility. You will assess, develop, implement, and monitor individualized nutrition plans in collaboration with the interdisciplinary team to ensure high-quality care and positive outcomes.

The role requires knowledge of skilled nursing regulations, experience with geriatric nutrition, and strong communication.

Qualifications

• Current Registered Dietitian (RD/RDN) credential required.
• Strong knowledge of skilled nursing nutrition regulations and compliance standards.
• Understanding of aging and nutritional needs of long-term care residents.
• Excellent communication, organization, and critical-thinking skills.
• Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

• Assess, develop, implement, and monitor individualized nutrition plans for residents.
• Collaborate with nursing, dietary services, physicians, and the interdisciplinary team.
• Monitor weight trends, labs, food intake, and overall nutritional status.
• Provide guidance on therapeutic diets, supplements, and nutrition interventions.
• Deliver nutrition education and counseling to residents, families, and staff.
