Job summary

The organization is seeking a dependable Physical Therapist Assistant to join a Skilled Nursing Facility in Concord, CA. You will work under the supervision of a licensed Physical Therapist to deliver hands-on care aimed at improving mobility, strength, and overall function for geriatric patients.

Responsibilities include implementing treatment plans, guiding exercises and gait training, and documenting patient progress while maintaining compliance with facility policies.

Qualifications

• Active state license as a Physical Therapist Assistant in California.
• Current BLS/CPR certification.
• Minimum of 6 months of PTA experience within the last 5 years preferred.
• Experience in a Skilled Nursing Facility or geriatric setting is a plus.
• Strong communication and teamwork skills.

Responsibilities

• Assist the Physical Therapist in implementing individualized treatment plans in a Skilled Nursing Facility setting
• Provide therapeutic exercises, gait training, and functional mobility activities
• Monitor patient progress and communicate any changes to the supervising Physical Therapist
• Educate patients and caregivers on exercises, safety, and proper mobility techniques
• Maintain accurate and timely documentation of treatments and patient progress
• Ensure compliance with facility policies and regulatory requirements
