Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant for home visits in Homosassa, FL. This PRN role offers flexible scheduling and patient-centered geriatric therapy to help residents age in place.

You will provide skilled treatments under a plan of care, support mobility and independence, and collaborate with physicians and families. Documentation must be precise and timely.

Qualifications

• Must have Florida PTA license and PTA degree.
• Two years clinical experience preferred.
• Must be able to travel to zip codes 33805 and 33801.

Responsibilities

• Provide skilled physical therapy treatments under the plan of care.
• Help patients improve mobility, function, and independence.
• Communicate with physicians, families, and the care team.
