Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant to provide geriatric therapy in home and assisted living facility settings. This PRN role offers flexible scheduling and the chance to work with an aging population to maintain independence.

Responsibilities include delivering skilled PTAs under the care plan, documenting promptly, and collaborating with physicians and families to optimize mobility and function in a home-based care model.

Qualifications

• Active Florida PTA license required.
• Two years of clinical experience preferred (not required).
• Licensed PTA to provide geriatric therapy in home/ALF settings.

Responsibilities

• Provide skilled physical therapy treatments under the plan of care.
• Help patients improve mobility, function, and independence.
• Communicate effectively with physicians, families, and the care team.
• Complete accurate, timely documentation.
