Sobe Innovative Rehab is seeking a licensed Physical Therapist Assistant (PTA) for PRN home visits in Palm Coast, FL. The PTA will provide skilled geriatric therapy, helping residents maintain independence and age in place, with payments per visit for Hospice and Part B services.

Responsibilities include implementing the plan of care, improving mobility, communicating with the care team, and documenting services accurately.

Competenze

• Active Florida PTA license is required.
• Associate degree from an accredited PTA program is required.
• Two years of clinical experience is preferred (not required).
