Job summary

FOX Rehabilitation in Chantilly, VA is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in private homes and senior living communities. You’ll develop individualized plans of care, perform evaluations, and utilize telehealth when appropriate to extend access.

Qualifications

• Licensed PT in the state of practice or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and function.
• Document effectively within FOX's EMR system per regulatory standards.
• Collaborate with PT, OT, SLP, and Fitness Specialists as part of an interdisciplinary team.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
