Job summary

Blue Ridge Care is seeking a Physical Therapist for its PACE program to support older adults in Center and home settings. You will perform evaluations, develop plans, and collaborate with an interdisciplinary team to maintain independence and mobility.

You will educate participants and caregivers, supervise PT staff, and ensure documentation, quality, and compliance across care settings. A valid PT license and CPR cert are required.

Qualifications

  • Graduate of an accredited Physical Therapy program is required.
  • Current Physical Therapist license is required.
  • Current CPR certification required.
  • Minimum of one year of frail/elderly population experience preferred.
  • Experience in interdisciplinary healthcare settings preferred.
  • Strong assessment, clinical reasoning and care planning skills.

Responsibilities

  • Conduct comprehensive PT assessments for new participants.
  • Develop and implement individualized treatment plans with the Interdisciplinary Team.
  • Provide PT evaluations, treatments, and interventions in multiple care settings.
  • Educate participants and caregivers on exercises, transfers, gait training, and safety.
  • Supervise and mentor PT Assistants and other staff; maintain documentation.
  • Support quality improvement initiatives and ensure regulatory compliance.

As a Physical Therapist with our PACE program, you'll play a vital role in helping older adults maintain independence, improve mobility, and achieve their highest level of function. Working as part of an interdisciplinary care team, you'll provide comprehensive physical therapy services across the PACE Center, participants' homes, and other care settings while supporting our mission of helping seniors remain safely in their communities.

About the Role

Under the supervision of the Director of PACE, the Physical Therapist provides evaluation, treatment, and ongoing therapy services to program participants. This position develops and implements individualized treatment plans, collaborates with the Interdisciplinary Team, oversees physical therapy support staff, and helps ensure participants receive coordinated, high-quality care that supports their health, safety, and independence.

How You'll Make a Difference
  • Conduct comprehensive physical therapy assessments for new participants upon enrollment.

  • Complete reassessments at least every six months, or more frequently as clinically indicated.

  • Collaborate with the Interdisciplinary Team to develop and implement participant-centered care plans.

  • Provide physical therapy evaluations, treatments, and interventions in the PACE Center, participant homes, nursing facilities, and community settings.

  • Establish individualized treatment goals and modify care plans based on participant progress and changing needs.

  • Educate participants and caregivers on exercise programs, mobility techniques, transfers, gait training, adaptive equipment, and home safety modifications.

  • Assess, recommend, and assist with the acquisition of durable medical equipment (DME), orthotics, prosthetics, and specialized footwear.

  • Evaluate newly issued equipment and devices to ensure proper fit and functionality.

  • Supervise, mentor, evaluate, and perform competencies for Physical Therapy Assistants and other designated staff.

  • Provide ongoing clinical education and support to rehabilitation team members.

  • Train staff on body mechanics, safe transfers, mobility techniques, and injury prevention strategies.

  • Maintain accurate documentation, participant statistics, and required reports.

  • Support quality improvement initiatives and continuous enhancement of care delivery.

  • Ensure compliance with applicable regulations, infection prevention standards, OSHA guidelines, and professional practice requirements.

  • Maintain equipment and therapy areas in a safe, clean, and organized condition.

  • Protect participant privacy and confidentiality in accordance with all policies and regulations.

  • Support Blue Ridge Care's mission, vision, and values through compassionate and collaborative care.

What You'll Bring
  • Graduate of an accredited Physical Therapy program required.

  • Current Physical Therapist license required.

  • Current CPR certification required.

  • Minimum of one (1) year of experience working with frail, elderly, or medically complex populations preferred.

  • Experience working in interdisciplinary healthcare settings preferred.

  • Strong assessment, clinical reasoning, and care planning skills.

  • Ability to effectively communicate with participants, families, providers, and interdisciplinary team members.

  • Knowledge of adaptive equipment, mobility training, rehabilitation practices, and fall prevention strategies.

  • Valid driver's license and reliable transportation required.

  • Successful completion of background screening required.

  • Must meet all health screening, immunization, and communicable disease clearance requirements prior to participant contact.

Physical Requirements
  • Ability to stand, walk, bend, lift, push, and pull throughout the workday.

  • Ability to lift and carry up to 50 pounds and safely push or pull equipment or objects exceeding 150 pounds using proper body mechanics.

  • Manual dexterity and hand-eye coordination required.

  • Ability to communicate effectively and accurately document participant care.

  • Ability to work in a fast-paced environment with changing priorities and participant needs.

  • Ability to work with frail older adults, including individuals with cognitive impairment or challenging behaviors.
