Therapy Management Corporation is seeking a licensed Physical Therapist to deliver patient-centered therapy to adult and geriatric populations in Terre Haute, IN. Qualifications

• Licensed Physical Therapist (PT) in the state of practice or license eligible.
• Graduate of an accredited Physical Therapist program.
• Interest or experience in skilled nursing, long-term care, or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized PT treatments to improve mobility, strength, balance, and functional independence.
• Develop and implement patient-centered treatment plans focused on long-term rehabilitation goals.
• Deliver hands-on therapy interventions to support recovery and enhance quality of life in older adults.
• Help patients restore functional mobility and safely perform daily activities.
• Partner with interdisciplinary teams, caregivers, and nursing staff to drive meaningful outcomes and continuity of care.
• Track patient progress, maintain accurate clinical documentation, and adapt plans as needed.
