Qualifications

• Licensed Physical Therapist in the state of practice or eligible.
• Graduate of an accredited Physical Therapist program.
• Interest or experience in skilled nursing or geriatric rehabilitation.
• Passion for patient-centered care and improving quality of life for older adults.

Responsibilities

• Evaluate patients and provide personalized physical therapy treatments.
• Develop and implement patient-centered treatment plans.
• Deliver hands-on therapy interventions for mobility and independence.
• Help patients restore functional mobility and daily activities.
• Collaborate with interdisciplinary teams and caregivers to drive outcomes.
• Track progress and maintain accurate clinical documentation.
