Job summary

Paramount Senior Living in Fayetteville, PA is seeking a Physical Therapist to evaluate residents and develop individualized treatment plans to help maintain independence, mobility, and quality of life. You will work within an interdisciplinary team in an Assisted Living and Skilled Nursing setting with a focus on resident-centered care.

Join our team to collaborate with nursing staff, educate residents and families on therapy goals, and maintain accurate therapy documentation.

Qualifications

• Active Pennsylvania Physical Therapist license required.
• Experience in short-term rehab or acute rehab preferred but not required.
• Strong communication and problem-solving skills.
• Ability to work independently and as part of a team.

Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Help residents improve mobility, strength, balance, and independence.
• Collaborate with nursing and care teams to support resident outcomes.
• Educate residents and families on treatment goals and progress.
• Maintain accurate documentation and therapy records.
