Job summary

Total Care Therapy is seeking a compassionate Physical Therapist to join a growing team serving assisted living and independent living communities in Toledo. You will evaluate, develop, and implement individualized plans to improve mobility, function and independence for older adults.

The role offers a therapist-owned culture, supportive leadership, and opportunities for clinical growth as the organization expands throughout Central Ohio.

Qualifications

• Current State Physical Therapist (PT) license
• Current CPR/BLS certification
• Valid driver's license with reliable transportation
• Experience in geriatrics, home health, assisted living, or skilled nursing preferred but not required

Responsibilities

• Evaluate residents and develop individualized plans of care.
• Provide skilled physical therapy treatments including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
• Reassess patient progress and modify treatment plans as needed.
• Supervise PTAs in accordance with state regulations.
• Complete timely and accurate documentation in the EMR.
• Educate residents, caregivers, and staff on plans, home exercise programs, fall prevention, and safe mobility techniques.
• Collaborate with interdisciplinary team for coordinated, patient-centered care.
• Maintain a productive clinical caseload while delivering quality therapy services.
