Proforma Health Inc. is hiring a Physical Therapy Assistant to support frail seniors in a PACE program, helping maintain safety, mobility, and independence.

You will implement therapist plans, lead exercises, gait and balance training, and educate caregivers. Visits occur in-center and at home, with thorough progress notes and teamwork with an interdisciplinary care team to enhance dignity and quality of life.

Formación

• Knowledge of therapeutic techniques, strong communication skills, compassionate approach, and ability to work collaboratively within a person-centered team environment. Focused on enhancing clients' dignity and quality of life through dedicated patient care.

Responsabilidades

• Implement treatment plans developed by Physical Therapists and lead therapeutic exercises.
• Conduct gait and balance training and instruct on safe use of assistive devices.
• Perform center-based and in-home visits and document progress accurately.
• Educate caregivers and participate in interdisciplinary team care planning.
