Tender Touch Rehab Services in Philadelphia is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. In this on-site role, you will implement PT treatment plans focused on improving mobility, function, safety, and quality of life.
Responsibilities include therapeutic exercise, gait and balance training, functional activities, accurate EMR
Formación
- Must hold a Certified Physical Therapist Assistant credential.
- Provide individualized therapy to seniors in skilled nursing and long-term care.
- Collaborate with an interdisciplinary team in a patient-centered culture.
- Accurately document care in the EMR and educate patients and caregivers.
Responsabilidades
- Implement PT treatment plans to improve mobility and safety.
- Perform therapeutic exercise, gait and balance training, and functional activities.
- Maintain accurate EMR documentation and patient education.
- Collaborate with the healthcare team to support quality of life for seniors.
