Job summary

Tender Touch Rehab Services seeks a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team. This on-site role delivers individualized therapy to seniors in skilled nursing and long-term care, implementing treatment plans that improve mobility, safety, and quality of life.

You will collaborate with an interdisciplinary team, perform therapeutic exercises and gait training, document accurately in EMR, and educate patients and caregivers.

Qualifications

• Must be a licensed Certified Physical Therapist Assistant (CPTA).
• Experience in geriatric therapy or long-term care preferred.
• Strong communication and patient education skills.

Responsibilities

• Provide individualized therapy to seniors in skilled nursing and long-term care settings.
• Execute PT treatment plans focusing on mobility, function, safety and quality of life.
• Perform therapeutic exercises, gait and balance training, and functional activities.
• Maintain accurate EMR documentation and educate patients and caregivers.
