Job summary

Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our on-site healthcare team serving seniors in skilled nursing and long-term care. You will implement PT treatment plans targeting mobility, function, safety, and quality of life.

Responsibilities include therapeutic exercise, gait and balance training, functional activities, EMR documentation, and education for patients and caregivers.

Qualifications

• Licensed PTA in good standing in the state of New Jersey.
• Experience in skilled nursing or long-term care is preferred.
• Proficiency with EMR and patient documentation is required.

Responsibilities

• Administer physical therapy treatments per approved plans.
• Conduct therapeutic exercises, gait and balance training, and functional activities.
• Maintain accurate EMR documentation of patient progress and interventions.
• Educate patients and caregivers to support ongoing outcomes.
