Job summary
Tender Touch Rehab Services is seeking a compassionate Certified Physical Therapist Assistant to join our Healthcare & Medical Services team, providing individualized therapy to seniors in skilled nursing and long-term care settings. This on-site role focuses on mobility, function, safety, and quality of life through targeted PT plans.
You will lead therapeutic exercise, gait and balance training, functional activities, and EMR documentation, while educating patients and caregivers.
Qualifications
- Must hold a Certified Physical Therapist Assistant credential.
- Experience providing therapeutic exercise and gait/balance training in senior care settings.
- Ability to document accurately in EMR and educate patients and caregivers.
Responsibilities
- Implement PT treatment plans to improve mobility, function, safety and quality of life for seniors.
- Perform therapeutic exercise, gait and balance training, and functional activities.
- Maintain accurate EMR documentation of patient progress and interventions.
- Educate patients and caregivers on care plans and home programs.
- Collaborate with an interdisciplinary team in a patient-centered culture.
