Eventus WholeHealth is seeking a Psychiatric Advanced Practice Clinician (NP or PA) to deliver behavioral health services in skilled nursing and senior living communities. You will conduct rounds, assess mental health conditions, and manage medications while coordinating with interdisciplinary teams to support residents' emotional well-being.

The role emphasizes proactive, value-based care, continuity of care with facility partners, and ongoing optimization of treatment plans.

Qualifications

• Current NP or PA licensure in the state of practice.
• Experience in geriatrics, family medicine, or psychiatry preferred.
• Exceptional patient-centered service skills, including compassionate communication and active listening.

Responsabilités

• Provide behavioral health and psychiatric medication management within skilled nursing and senior living communities.
• Perform routine patient rounding and evaluation with ongoing mental health management.
• Collaborate with interdisciplinary teams to support residents’ emotional and cognitive well-being.
• Ensure continuity of care, optimize treatment plans, and promote a therapeutic environment.
