Job summary

LeoForce in Anaheim, CA is seeking a Physician in Geriatric Primary Care to provide comprehensive, patient-centered primary care for seniors. You will lead chronic disease management, preventive care, and care coordination within an interdisciplinary team.

Board-certified/eligible in Family or Internal Medicine, CA license, DEA, and BLS are required. The role offers competitive compensation ($250k–$300k), a Monday–Friday schedule, no weekends, and strong benefits.

Qualifications

• Board certification or eligibility in Family Medicine or Internal Medicine.
• Active California physician license and DEA registration.
• BLS certification is required.
• Experience with geriatrics, primary care, skilled nursing or long-term care is preferred.

Responsibilities

• Provide primary medical care including assessments, treatment planning and ongoing care management.
• Diagnose and manage acute and chronic conditions in an elderly, complex population.
• Collaborate with nurses, specialists and the interdisciplinary team to coordinate care.
• Order and interpret diagnostics and adjust treatment plans as needed.
• Educate patients and families on disease management and preventive care.
• Supervise and collaborate with nurse practitioners and physician assistants.
• Maintain accurate and timely clinical documentation.
