Job summary

HarmonyCares in Chicago is seeking a Nurse Practitioner for primary care focused on geriatric patients. You will manage chronic conditions with 30–45 minute visits, Monday through Friday, with no weekends or evenings. Athena EMR and AI Scribe will help chart during the workday.

We require NP licensure in the state, DEA license/eligibility, CPR/BLS, and board certification. Geriatric experience is preferred and a valid driver's license is required.

Qualifications

• Licensed as an NP in the state where HarmonyCares Medical Group office is located.
• Federal DEA License/eligibility.
• Geriatric training/experience preferred.
• Active CPR/BLS Certification.
• Board Certification.
• Must maintain a valid driver’s license and good driving record.

Responsibilities

• Provide geriatric primary care and manage chronic conditions in alignment with HEDIS/STAR quality outcomes, focusing on patient needs over volume.
• Provide primary care to high-acuity patients in their home, Monday–Friday.
