Job summary

AllCare.ai seeks Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across partner communities. This full-time role offers steady patient volume and the opportunity to deliver high-quality care where seniors live.

You will conduct routine and urgent assessments, manage chronic diseases, coordinate with facility staff and families, and complete same-day documentation to support CMS compliance.

Qualifications

• Active California NP license and RN license, with furnishing license.
• NPI, DEA, and BLS certifications are required.
• Geriatrics or facility-based care experience preferred; EMR proficiency.

Role Overview

We're seeking Primary Care NPs to provide on-site care to Assisted Living/RCFE residents across our partner communities. This role offers steady, predictable patient volume and the opportunity to deliver high-quality care directly where seniors live.

What You'll Do

• Routine & urgent assessments; chronic disease management
• Post-discharge follow-ups, med reconciliation, labs/diagnostics
• Coordinate with facility staff, specialists, families/POAs
• Same-day documentation; CMS/regulatory compliance

Schedule & Setting

Full-time position with a structured weekday schedule. You'll spend 3 days delivering on-site care with strategically clustered routes designed to minimize travel and maximize time with patients. The remaining 2 days are in-office at our Irvine headquarters for administrative and documentation tasks.

Compensation

• Annual compensation range: $140,000-$160,000 (based on experience, schedule, and patient volume)
• Additional earning opportunities available based on coverage and panel needs
