Job summary

i4 Search Group Healthcare Recruiting is seeking a Primary Care Nurse Practitioner (NP) to join our team in Campbellsville, KY. This full-time role focuses on delivering high-quality primary and preventive care to residents in skilled nursing and long-term care facilities.

You will work with physicians, nursing staff, and interdisciplinary teams to coordinate patient care, manage chronic conditions, and support geriatric patients in a compassionate, patient-centered environment.

Qualifications

• Current board certification as a Nurse Practitioner.
• Active DEA registration or ability to obtain one.
• Clinical background in adult, family, or geriatric medicine.
• Minimum of one year of clinical experience preferred.
• Experience in skilled nursing, long-term care, or post-acute care settings preferred.
• Strong clinical assessment, diagnostic, and patient management skills.
• Proficiency with electronic medical records.
• Excellent communication, organizational, and collaborative skills.
• Behavioral health or psychiatric experience is a plus.

Responsibilities

• Provide comprehensive primary, preventive, and ongoing medical care to residents of skilled nursing and long-term care facilities.
• Evaluate, diagnose, and manage acute illnesses, chronic conditions, and changes in patient health status.
• Develop and implement individualized treatment plans, including chronic disease management and advance care planning.
• Prescribe medications, including controlled substances, in accordance with Kentucky regulations and applicable prescribing authority.
• Order, interpret, and follow up on laboratory tests, diagnostic studies, and medical treatments.
• Collaborate with physicians, nursing staff, and other healthcare professionals to coordinate patient care.
• Communicate regularly with facility staff regarding patient conditions, treatment recommendations, and care plans.
• Maintain accurate and timely clinical documentation using electronic medical records (EMR).
• Promote preventive healthcare, patient education, and improved quality of life for residents.
