Job summary

VIPcare in New York seeks a Nurse Practitioner to provide primary care for patients 65+ under physician supervision, using sound medical judgment and current guidelines.

You will manage acute and chronic conditions, coordinate care, document in EMR within 48 hours, and work with hospitalists when patients are admitted. Candidates must hold NP licensure, ACLS/BLS, and have EMR experience; strong communication and patient-focused service are essential.

Qualifications

• Graduate of an accredited school of nursing.
• Certified as a nurse practitioner with current state licensure.
• At least 1 year of nursing experience with 1–3 years as a primary care NP.
• Experience with electronic medical records (EMR).
• ACLS/BLS certification as mandated by state.

Responsibilities

• Practice medicine under supervision with sound medical judgment and ethics.
• Provide care for older adults with acute and chronic illness.
• Seek consult from specialists when needed and stay involved in patient care.
• Engage with hospitalists when a patient is in the hospital.
• Meet regulatory and safety requirements and document compliance.
• Assess, diagnose, treat, and coordinate care using evidence-based guidelines.
• Maintain complete EMR documentation within 48 hours of visits.
• Communicate effectively with patients, families, and team members.
