Job summary

Los Angeles Jewish Health is seeking a Physician for the Brandman Centers for Senior Care (PACE) in Redondo Beach. The position offers a salary range of $250,000 to $275,000 per year and a full-time schedule, Monday through Friday.

The physician will provide primary care to seniors, coordinate with an interdisciplinary team, and participate in quality improvement and program goals within the BCSC PACE model.

Qualifications

• Graduation from an accredited School of Medicine.
• Board certification in Internal Medicine or Family Practice.
• Geriatric medicine certification is preferred.

Responsibilities

• Performs pre-enrollment/initial assessments and 6-month evaluations with comprehensive history and exams.
• Develops and implements a Plan of Care at least quarterly and with condition changes.
• Oversees smooth transitions into BCSC by coordinating with prior providers.
• Provides evaluation, treatment and ongoing monitoring of episodic illness.
• Provides primary care to participants and coordinates hospital admissions as needed.
• Participates in Interdisciplinary Team meetings and quality improvement activities.
