Job summary

California Park Post Acute is seeking a licensed Physical Therapist to provide and direct comprehensive patient care in a skilled nursing setting in Chico, California. You will evaluate patients, develop and implement treatment plans, educate families and staff, and ensure accurate documentation in accordance with regulatory guidelines.

The role requires active California PT licensure, CPR certification, and knowledge of Medicare/payor sources.

Qualifications

• Graduate of an approved Physical Therapy curriculum.
• Current, active Physical Therapist license.
• CPR certification as required for license renewal.
• Knowledge of Medicare and other payor sources.
• Full knowledge of resident rights.

Responsibilities

• Directs and supervises assistants and rehab staff in accordance with state and federal guidelines.
• Develops and implements treatment plans to achieve maximal functional outcomes.
• Educates patients, families, and staff on therapy goals, progress, and safety.
• Completes documentation including evaluations, daily progress notes, and monthly summaries.
• Screens individuals who may benefit from therapy and refers to appropriate team members.
• Maintains licensure, credentials, and participates in continuing education.
