Career Links is seeking a compassionate Physical Therapist (PT) for a Skilled Nursing Facility setting. The role involves evaluating residents, designing individualized treatment plans, and guiding therapy to restore mobility and independence.

Qualifications

• Current Iowa Physical Therapist license.
• Graduate of a CAPTE-recognized PT program.
• Eligible for or holding certification through the Federation of State Boards of Physical Therapy.
• Strong communication and documentation.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans
• Provide therapeutic interventions including strength training, balance training, gait training, and mobility exercises
• Assist residents in improving functional mobility, transfers, and ambulation
• Collaborate with interdisciplinary care teams including OT, SLP, nursing staff, and physicians
• Educate residents and caregivers on safety, fall prevention, and home exercise programs
• Participate in care planning meetings and discharge planning
