Job summary

Creekside Healthcare Center is seeking a Physical Therapist (PT) to evaluate and treat residents and patients with temporary or long-term physical disabilities. You will relieve pain, restore function, and promote healing through exercise, heat, ultrasound, massage, and other treatments.

Under physician orders, you will assess and treat neuromuscular and musculoskeletal issues, interpret tests, and guide patients and families.

Qualifications

• Bachelor/masters degree in Physical Therapy required.
• 1 year of clinical experience preferred.
• Current license to practice Physical Therapy in California.
• Covid-19 vaccine required.
• Must comply with California Covid mandate.

Responsibilities

• Evaluates, plans and administers treatment for neuromuscular and musculoskeletal issues.
• Tests and measures muscle strength, ROM, and respiratory/circulatory efficiency.
• Provides direct resident treatment per care plan.
• Educates patients and caregivers on devices and mobility aids.
• Maintains patient records and documents.
• Develops and applies patient-specific treatment tools.
• Performs home assessments to support safe discharge.
