Job summary

Creekside Post-Acute is seeking a dedicated Physical Therapist (PT) to evaluate residents and develop individualized treatment plans in a skilled nursing setting in San Jose, CA. You will deliver targeted therapy to improve mobility, balance, and independence, while collaborating with an interdisciplinary team and families to support resident goals.

The role requires a DPT, California PT license, and experience in post-acute or geriatric rehabilitation.

Qualifications

• Earn a Doctor of Physical Therapy (DPT) degree from an accredited physical therapy program.
• Pass both national and state board examinations required for licensure.
• Hold a current and unrestricted California Physical Therapist license.
• Previous experience in Skilled Nursing, post-acute care, rehabilitation, or geriatric settings preferred.

Responsibilities

• Conduct comprehensive physical therapy evaluations based on physician orders, resident needs, and clinical findings.
• Develop individualized treatment plans with measurable goals based on each resident's functional needs.
• Provide skilled physical therapy interventions to improve strength, mobility, balance, coordination, endurance, and functional independence.
• Provide therapeutic exercises, gait training, transfer training, mobility activities, and other appropriate interventions.
• Monitor resident progress and modify treatment plans as clinically appropriate.
• Educate residents, families, and caregivers regarding exercises, mobility, safety, and strategies to support continued progress.
• Maintain accurate, timely, and complete clinical documentation.
• Participate in care plan meetings and collaborate with the interdisciplinary team.
• Communicate effectively with nursing, physicians, occupational therapists, speech therapists, rehabilitation staff, residents, families, and caregivers.
• Promote resident safety, dignity, independence, and quality of life.
• Follow facility policies, professional standards, and applicable federal and state regulations.
• Maintain a safe and therapeutic treatment environment.
• Perform other duties as assigned.
