Job summary

CareRite Centers, LLC is seeking an Experienced Physical Therapist (PT) for our Chatham Hills Subacute Care Center. The PT will assess patients requiring rehabilitation, establish and manage treatment programs, and educate families on therapy methods.

The role includes supervising Physical Therapy Assistants, documenting progress, and collaborating with other disciplines to deliver patient-centered care in a subacute setting. Prior geriatric care experience is preferred.

Qualifications

• Graduated from an accredited Physical Therapy college or university.
• Licensed and registered to practice physical therapy in the state.
• Oversee and supervise Physical Therapy Assistants.
• Experience in long-term or geriatric care preferred.
• Strong documentation writing skills.
• Ability to document evaluations, progress notes, recertifications and discharge summaries.
• Collaborates with all disciplines to provide patient-centered care.

Responsibilities

• Assess referred patients needing rehabilitation and provide direct patient care.
• Develop and administer treatment programs with goals based on patient capacity.
• Document patient condition and progress; educate patients and families.
• Set realistic goals and monitor progress under physician direction.
• Respond professionally and maintain confidentiality and HIPAA compliance.
