The Legacy Senior Communities is seeking a licensed Physical Therapist to plan, organize, and implement a progressive therapy program aligned with residents’ physical, psychological, and social well-being. The role supervises therapy assistants and adheres to regulatory standards.

Responsibilities include assessment, planning, implementation, evaluation, and discharge planning, with strong collaboration across the care team.

Formación

• Bachelor of Science or Master’s Degree from an APTA accredited program.
• Texas license to practice as a Physical Therapist.
• Fieldwork experience in geriatrics or hospital acute/sub-acute/rehab.
• CPR certification (AHA/Red Cross) with hands-on practice and in-person skills assessment within 30 days of hire.

Responsabilidades

• Assess patients upon PT orders and review history, completing thorough assessment.
• Screen patients for intervention and communicate results to patient/family/staff.
• Develop and prioritize treatment plans with goals in collaboration with the health care team.
• Implement interventions per plan of care and adjust based on patient response.
• Document daily interventions and reassess patient progress for discharge planning.
• Maintain confidentiality and comply with HIPAA and facility policies.

Educación
