Job summary

Habitat Health in South Los Angeles is seeking a licensed Physical Therapist to join our PACE program in a part-time capacity. The role involves 16–24 hours per week with schedule to be set at offer.

The PT will provide evaluation and treatment across acute, restorative, and maintenance care, develop plans, and educate participants and caregivers. A California PT license and CMS-aligned experience are preferred.

Qualifications

• Bachelor's degree in Physical Therapy.
• Completion of an APTA-accredited physical therapy program or equivalent approved by CMS.
• Active California Physical Therapist license.
• Minimum of one year experience with frail or elderly.

Responsibilities

• Provide PT evaluation and treatment across acute, restorative, and maintenance levels for PACE participants.
• Develop individualized treatment plans and conduct home visits.
• Educate participants and caregivers on exercises and DME use.
• Participate as core IDT member for assessments and care coordination.
