Job summary

Habitat Health is seeking a Physical Therapist to deliver comprehensive PT evaluation and treatment for PACE participants, including home visits and caregiver education. You will work as part of the Interdisciplinary Team to develop plans, coordinate care, and ensure documentation complies with regulations.

The role requires a California PT license, one year in elderly care, and CPR/BLS. Compensation includes a base salary in the $128k–$145k range and a full benefits package with health,

Qualifications

• Bachelor’s Degree in Physical Therapy from an accredited program.
• Active Physical Therapist license issued by California.
• Minimum of one year experience with frail or elderly individuals.
• Current CPR/BLS certification.
• Ability to thrive in a fast-paced, evolving environment and use data to inform decisions.

Responsibilities

• Provide PT evaluation and treatment across acute, restorative, and maintenance levels for PACE participants.
• Develop individualized treatment plans and conduct home visits as needed.
• Educate participants and caregivers on exercise programs and DME use.
• Contribute to IDT care planning and document in line with regulatory requirements.
• Assist in DME inventory management and ensure infection control standards.
