Job summary

Habitat Health in South Los Angeles is seeking a licensed Physical Therapist to join our PACE program in a part-time capacity. The role involves 16–24 hours per week with schedule to be set at offer.

The PT will provide evaluation and treatment across acute, restorative, and maintenance care, develop plans, and educate participants and caregivers. A California PT license and CMS-aligned experience are preferred.

Qualifications

  • Bachelor's degree in Physical Therapy.
  • Completion of an APTA-accredited physical therapy program or equivalent approved by CMS.
  • Active California Physical Therapist license.
  • Minimum of one year experience with frail or elderly.

Role Scope

Role: Physical Therapist to join Habitat Health at our South LA PACE Center in a part-time capacity. The ideal candidate will work 16-24 hours per week. Specific schedule to be determined at offer.

Responsibilities
  • Provide physical therapy evaluation and treatment across acute, restorative, and maintenance levels of care for PACE participants.
  • Develop individualized treatment plans and conduct home visits as needed.
  • Educate participants and caregivers on exercise programs and DME use.
  • Participate as a core member of the Interdisciplinary Team (IDT) assisting with clinical assessments and care coordination to inform whole-person care planning; maintain documentation meeting regulatory requirements.
  • Serve as a subject matter expert for PT program standardization, team education, and quality audits; support DME inventory management; uphold safety and infection control standards.
Administrative & Program Development
  • Improve clinical outcomes and participant experience through feedback and process improvements.
  • Assist in ordering, inventory, distribution, and maintenance of durable medical equipment (DME).
  • Implement the physical therapy program, complete evaluations and care plans timely; oversee group maintenance exercises with rehabilitation aides.
  • Maintain quality assurance, safety, environmental, and infection control programs.
  • Collect data, prepare reports, and maintain participant statistics; participate in staff meetings and patient care conferences.
Required Qualifications
  • Bachelor’s Degree in Physical Therapy
  • Completion of APTA-accredited physical therapy program or equivalent approved by CMS.
  • Active Physical Therapist license issued by California.
  • Minimum of one year experience working with the frail or elderly.

Nice to Have
  • 2+ years as a practicing PT
  • Doctorate of Physical Therapy (DPT)
  • Bilingual: Spanish/English (strongly preferred); other languages a plus
  • PACE experience and/or ADCH/CBAS experience
  • State-issued driver’s license, personal transportation, and auto insurance as required by law

Compensation

The anticipated base salary range for this position is $69.00 to $77.00 per hour. The final compensation offered will be determined based on factors including qualifications, skills, experience, education, certifications, location, and business needs.
